Re: “Code Blue” Grant Request and Authorization

Attached is the Code Blue EMS Equipment Request Forms I wish to send in to Southern Region EMS Council for possible funding in the next round of Code Blue grants. Any funding received will require authorization of up to a 20% match from the City. I estimate that that match will amount to $2,340. The final amount will be determined when the grant source is approved and the currently market price of the items requested. The amount requested is based on today’s prices which may or may not be accurate at the time of the grant award (up to 18 months from submission). I would not expect more than a 10% price increase in that time frame. In order to process the grant request Southern Region EMS Council requires all submission to be accompanied by a letter from the governing body of the organization verifying that a match is approved and will be funded if the grant is awarded.
