Commercial Permit Holder Agreement

Commercial User is defined as a corporate entity or business owner who authorizes individuals to operate the cranes on their behalf.  For example:  a vessel owner that authorizes crew members to operate a City-owned crane on vessel owner’s behalf.  Commercial entities must complete this form and follow the steps below to be authorized to operate the City cranes.

  1. Submit Paperwork at Port and Harbor Office
    a.)  A Commercial/Permit Holder Crane Use Agreement and Indemnification must be completed and signed at the Port and Harbor Office.  Commercial User Agreements expire after 3 years.  A sample Commercial User Crane Agreement and Indemnification is posted below.

b.)  Provide Proof of General Liability Insurance that includes crane use.  Insurance will have limits of liability not less than $1,000,000 combined, single limit bodily injury and property damage, $1,000,000 personal injury, and $2,000,000 aggregate.  The policy shall name the City of Homer as an additional assured.  Further details can be found in the Commercial User Agreement below.

c.)  Port and Harbor Office Administrative Staff will issue new Crane Access Card(s) (plastic), or activate existing card(s) under the new requirements (new expiration date).

  1. Ensure Training of Operators – Commercial User shall ensure that all operators under their permit complete individual online and proficiency trainings, sign Covered Operator Agreements, and receive an Operator card (paper) at the Fish Dock.  Please see Covered Operator Agreement for form and instructions on how to obtain authorization for individuals operating under entity's Commercial Permit.
